ARA-290, also known as cibinetide, is a synthetic 11-amino-acid peptide derived from the helix B surface domain of erythropoietin (EPO). Unlike EPO itself, ARA-290 is non-erythropoietic: it does not bind the classical homodimeric EPO receptor and does not stimulate red blood cell production. Instead it selectively engages the innate repair receptor (IRR), a heterocomplex of the EPO receptor and the beta-common receptor (CD131). This separation of tissue-protective signalling from haematopoietic signalling is what makes ARA-290 a widely used tool compound in cytoprotection research.
Innate Repair Receptor Signalling
The IRR is expressed on injured and metabolically stressed tissue rather than healthy tissue, which gives researchers a route to study repair signalling with spatial selectivity. Common lines of investigation include:
– Receptor Characterisation: Investigating EPOR/CD131 heterocomplex assembly, ligand selectivity, and how IRR engagement differs from classical EPO receptor activation in cellular models.
– Downstream Pathways: Studying JAK2/STAT3, PI3K/Akt and NF-κB signalling following IRR activation, and the resulting changes in pro- and anti-inflammatory gene expression.
– Cytoprotection Models: Examining tissue-protective responses in ischaemia–reperfusion, metabolic stress and inflammatory injury preparations.
Neuropathy and Small-Fibre Research
Much of the published ARA-290 literature concerns peripheral nerve models, making it a reference compound for researchers in this area:
– Small-Fibre Neuropathy: Investigating corneal nerve fibre density and intraepidermal nerve fibre measures as structural readouts in neuropathy models.
– Neuropathic Pain Pathways: Studying nociceptive signalling and neuroimmune interaction, including glial activation and cytokine involvement.
– Metabolic Neuropathy: Examining nerve function in diabetic and metabolically stressed preparations.
Inflammation and Immune Modulation
– Cytokine Profiling: Measuring TNF-α, IL-6 and IL-1β responses following IRR engagement in immune cell cultures.
– Macrophage Polarisation: Investigating shifts in macrophage phenotype and resolution-phase signalling.
– Granulomatous Inflammation: ARA-290 features in sarcoidosis-associated neuropathy literature, providing a comparator for inflammation-resolution research.
